A Biomechanical Analysis of Shoulder Stabilization: Posterior-Inferior Glenohumeral Capsular Plication

چکیده

INTRODUCTION The use of posterior capsular plication to decrease capsular volume and reduce capsular laxity for treatment for posterior instability, multidirectional instability, or as an additional technique in the treatment of anterior instability is common. Although it is well known that capsular stitches hold well in the labrum and anterior capsule, the posterior capsule is thinner, less robust biomechanically, and may not provide an optimal fixation construct. While multiple suturing techniques have been described, relatively little is known regarding the biomechanical stability afforded by various approaches. The purpose of the present study was to determine the biomechanical properties of suture plication of the posterior-inferior quadrant of the glenoid, performed using various repair constructs to an intact labrum.

A biomechanical analysis of shoulder stabilization: posteroinferior glenohumeral capsular plication.

BACKGROUND The use of posterior capsular plication to decrease capsular volume and address capsular laxity for treatment of posterior instability, multidirectional instability, or as an additional technique in the treatment of anterior instability is common. Multiple different suturing techniques have been described. HYPOTHESIS The simple stitch will have inferior biomechanical properties com...

[Arthroscopic treatment of anterior-inferior shoulder instability].

OBJECTIVES We evaluated patients who underwent arthroscopic repair for posttraumatic, recurrent anterior-inferior glenohumeral instability with capsular laxity. METHODS Seventeen patients (4 females, 13 males; mean age 27 years; range 18 to 40 years) were treated with arthroscopic Bankart repair and posterior capsular plication for posttraumatic, recurrent anterior-inferior glenohumeral insta...

Posterior Capsular Plication Constrains the Glenohumeral Joint by Drawing the Humeral Head Closer to the Glenoid and Resisting Abduction

BACKGROUND Shoulder pain is a common problem, with 30% to 50% of the American population affected annually. While the majority of these shoulder problems improve, there is a high rate of recurrence, as 54% of patients experience persistent symptoms 3 years after onset. PURPOSE Posterior shoulder tightness has been shown to alter glenohumeral (GH) kinematics. Clinically, posterior shoulder con...

The Rotator Interval of the Shoulder: Implications in the Treatment of Shoulder Instability.

Biomechanical studies have shown that repair or plication of rotator interval (RI) ligamentous and capsular structures decreases glenohumeral joint laxity in various directions. Clinical outcomes studies have reported successful outcomes after repair or plication of these structures in patients undergoing shoulder stabilization procedures. Recent studies describing arthroscopic techniques to ad...

Biomechanical evaluation of multidirectional glenohumeral instability and repair.

The purpose of the current study was to create a multidirectional glenohumeral instability model and compare anterior capsulolabral reconstruction with inferior capsular shift with respect to their effects on glenohumeral translation and rotational range of motion. Ten fresh frozen cadaveric shoulders were used with a custom shoulder translation testing jig. To create the multidirectional insta...
